How to Sign up for Social Security Disability
The Social Security Administration (SSA) determines your eligibility for disability benefits based on your work history and your inability to work due to a disease, injury or illness. You might be eligible if your disability has lasted or will last for more than a year. Your dependents can also receive benefits on your record, which will usually total about 50 percent of your benefit. - Telephone
- Transportation to your local SSA office (for in-person interview)
- W-2 or Form 1040 from the past year
- Medical records

Apply online by navigating to the SSA website at SSA.gov and clicking on "Disability."

Prepare for your application by reviewing the checklist. It contains items you'll need such as a DD214 for a military discharge, a W-2 for employment or a 1040 Form for self-employment from the past year.

Complete the online application.

Complete the "Adult Disability Report," which has seven sections. Each section has a set of instructions and an example.

Print the "Authorization to Disclose Information to the Social Security Administration." Mail or take it to your local SSA office.
