How to Get Disability in Arizona
No matter what state you reside in, the process for receiving disability benefits is the same. The Social Security Administration, which distributes disability benefits, determines whether or not an individual is eligible to receive those benefits. The SSA leaves the investigation of your case and the ultimate decision of an individual's application up a local state agency (in Arizona, it's Disability Determination Services).Instructions

1
Determine whether or not you are eligible to receive disability benefits from the SSA. To do so, you must meet earnings criteria that include a "recent work test" and a "duration of work" test, both of which judge how long you have worked related to your age.
-
2
Initiate an application for disability benefits immediately after becoming disabled, as the process can take months. Complete the SSA's Disability Report to accompany your application. Gather supplementary information related to doctors you have visited, test results, medical records, federal tax returns or any other information that may be pertinent to your application.

3
Complete other forms that give doctors and hospitals permission to release your medical information, as well as another supplemental form detailing how your medical condition affects your ability to work. Get these forms from the SSA's website (see Resources) or by visiting your local SSA office in Arizona.

4
Return the completed application online via the SSA's electronic form or visit a local SSA office in Arizona. Once the SSA has determined that you meet basic requirements, your application will be sent to an Arizona Disability Determination Services office for review.

Wait for up to five months for a decision to be made regarding your application. If you're approved, you'll get a letter stating the amount of the benefits and when payments will begin. If you're not, you'll receive a letter with information about how to appeal.
