How to Obtain a Disability Parking Permit

      Visit your doctor. Your physician will be required to fill out the "Disability Statement" portion of your parking permit application. Bring your application to your doctor and ask him to the complete the form for you.

      Complete the rest of the application. You will be required to provide your name, address, phone number, and vehicle information.

      Submit the application with payment. Your application should be submitted to your local tax collector's office. There will be an application fee, which varies by state. You will also need to bring a driver's license or other proof of identity.
