Gather your documents. You need three documents to prove Ontario residency: one that shows you are a Canadian citizen; one that shows you are an Ontario resident and one that proves your identity. Gather any additional documents, such as a marriage certificate or a change of name certificate. Look at the official form in Resource 1 for a complete list of acceptable documents.

Go to an Ontario Health Insurance Provider office to apply for a replacement card.

Explain that you need a replacement health card. Provide all your documents to the staff person at the desk. Answer any questions. The staff person will fill out the necessary forms for you.

Accept the new card if the office is able to print one out for your. Otherwise, wait for the new card. It should arrive in the mail within a few days to a week.
