How to Apply for PeachCare for Kids

In the United States, over eight million children do not have health insurance and many more do not have adequate health coverage, according to the Children's Defense Fund. If your child does not have insurance and also does not qualify for Medicaid, you can apply for PeachCare for Kids. PeachCare for Kids provides comprehensive health care coverage to children living in Georgia. This program covers preventative care, such as check-ups and vaccinations, as well as emergency services and prescription medicine coverage.

  Check the eligibility requirements for PeachCare. You and your child must live in Georgia and your child must be under the age of 18. Your child must also be uninsured and you must meet income requirements. As of 2011, the maximum income of a family of three is $43,546. Call PeachCare at 404-463-8370 for current eligibility requirements.

  Go to the PeachCare for Kids website and click "Apply Now."

  Click on "Apply Now" on the next web page to complete an online application. Alternatively, click on "Download Application" in either English or Spanish to fill out a printed application and mail it in.

  Create an account with PeachCare if you wish to fill out an online application. Enter a user name and password, along with your e-mail address, security question and answer. Click "Register."

  Fill out all information on either the online or printed application. Enter your basic information, such as your social security number and address. Include the information of the second parent, if applicable. Enter the information of your child or children, including social security numbers and insurance information, if applicable. Fill out all of your income information, including wages, child support, benefits and any unearned income.

  Follow the onscreen instructions to submit your online application. Alternatively, place the printed application in an envelope and address it to "PeachCare for Kids, P.O. Box 2583, Atlanta, GA 30301-2583."

  Gather together the required documents, as indicated on the online or printed application. This includes one month of pay stubs or self employment documents, as well as proof of child support and any benefits. Mail these documents to PeachCare for Kids along with the application, if you printed it.
