Medicare is the U.S. government provided health insurance program for retirees over age 65. Your Medicare card provides information concerning the types of Medicare coverage you are entitled to and health care providers will ask for the card when you request medical care. You can obtain a new card through the Social Security website.

      Locate the Medicare card replacement page on the Social Security website at socialsecurity.gov/medicarecard/. You will find a page titled: "Information about Replacing a Medicare Card". Scroll to the bottom of the page and click the "Start" button.

      Click the "I Agree" button on the privacy statement page. You will be taken to the Web page to start the application for a new Medicare card.

      Enter all of the requested information. You will need your last statement of Medicare payments to provide recent payment information as part of the identity verification.
