      Call the Oklahoma Insurance Department's consumer assistance division at 800-522-0071 for help with your insurance issues. If you need help with buying or understanding your insurance or want to discuss an issue, representatives can help you.

      File a formal complaint with the Oklahoma Insurance Department by filling out the online form, including very detailed information about your issue. You can submit the form online or mail it with any supporting documentation to the address on the form (see Resources). You also can fax it to 405-521-6652. Provide the department with a copy of your policy, any correspondence you've received from your insurance company and a record of your contacts with the insurer.
