How to Renew an OHIP Card

If your Ontario Health Insurance Plan or OHIP card is set to expire, learning how to renew your health insurance will ensure your coverage continues uninterrupted. While you do not need to renew the red and white version of the OHIP card, the photo version requires a visit to your nearest OHIP office. Save time by booking an appointment in advance, where possible, and know what steps you need to follow so that the renewal process goes smoothly.

      Examine the back of your OHIP card and locate the expiry date. Two months before your renewal date, you will receive a renewal notice in the mail from the Ontario Ministry of Health and Long-Term Care. If you have not received a renewal notice, and it is no more than six months before your expiry date, you can renew your card by visiting the closest OHIP office.

      Find the closest OHIP office to your work or home. If you are not sure where the closest office is located, visit ServiceOntario.ca.

      Click on "Find Services and Locations." This will take you to a new webpage. Select "Photo Health Card Renewal" under Most Popular Services.

      Under the Service Counter tab, click on "Find a Counter," enter your city or postal code and click "Find Location." Write down the address, directions and hours of the closest location.

      Visit the Ontario Ministry of Health Long-Term Care website. Click on "Public Information" and then "Forms" on the left side of the page. Next, select "Ontario Health Insurance," which will take you to a new window. Here, click on the form titled, "Health Card Renewal." Complete and sign the form.

      Gather all the documents and cards that you will need to bring with you. This includes the completed health card renewal form, your health card, proof of residency in Ontario such as a bank account statement or utility bill, proof of citizenship such as a birth certificate or passport, and proof of identity such as a driver's license or credit card.

      Visit your closest OHIP office to renew your health card.
