How to Request a New Medical ID Card
Mistakes in life happen, and sometimes you misplace or lose your current medical ID card. Requesting a new ID card is not difficult. There are different procedures based on the type of card lost, such as a VA card, private or group health insurance card and Medicaid or Medicare card. Once you obtain a new card, keep it in a safe place to avoid repeating the replacement process.Things You'll Need

Visit your local VA facility to obtain a new VA card. Bring a photo ID and additional proof of eligibility such as a Certificate of Discharge. Your photo will be taken, and the new card will be mailed to you within five to seven days.

Contact your insurance agent to request a new private or group health insurance card. Agents will have access to order a new card for you. You can also contact your health insurance company directly by locating the number found on your insurance policy. Verify your security information, such as a current address, maiden name, if applicable, and social security number.

Request a new Medicaid card through your independent caseworker. In general, new Medicaid cards are mailed out annually. If you lose or misplace your card prior to your card anniversary, then your caseworker will be able to request a new one. New cards may take up to four weeks to arrive.

Obtain a new Medicare card one of two ways. Online ordering is available through the Medicare website. Fill out the exact last payment amount, or the last month and year payment was made. Identify your name as it appears on the card, your social security number, your date of birth and your current phone number. New Medicare cards are mailed only to the current address on file and may take up to four weeks to arrive. You can also call 1-800-772-1213 to request a new card.
