How to Replace Tricare Prime Cards

Tricare Prime is a medical insurance option for members of the United States military that are currently on active duty, or activated Guard and Reserve members, as well as their dependents. Tricare Prime offers members the opportunity to select a primary care physician from the Tricare network, which allows them to incur no out -of-pocket-costs as long as they use that specified physician and other medical providers referred by the primary care physician.

      Determine which Tricare region you are located and enrolled in: North Region, South Region or West Region.

      Gather the following information: Sponsor's first and last name, sponsor's social security number and the names and birth dates of all beneficiaries who need replacement cards.

      Contact your regional provider customer service hot line over the phone. You can find the number on your previous Tricare Prime card, or on the Tricare Prime website.

      Follow the automated prompts until you reach a customer service representative.

      Inform them of your need to obtain a replacement card. The representative will request your sponsor's name and social security number, the name and birth dates of all beneficiaries who require replacement cards, your mailing address and a phone number where you can be reached.

      Tricare Prime members may also request replacement cards online, by visiting the Enrollment section of their respective regional provider's website, and filling out a form for a replacement card. The form will require that you input the same information that would be asked of you over the phone.
