How to Qualify for Oregon Insurance
The Oregon Health Plan covers a wide range of services, including the diagnosis of illnesses, check-ups (medical and dental), family planning, alcohol/drug treatment and mental health. If you are an Oregon resident, you may qualify for coverage. Coverage is based on your income, age and physical or mental condition. For example, pregnant women and children receive the most coverage under the Oregon Health Insurance Plan.Instructions

Check eligibility. According to the Oregon Department of Human Services, different eligibility rules and income levels apply. Visit the Oregon Helps website to see if your family qualifies for assistance through the Oregon Health Plan.

Complete the Oregon Helps' pre-screener questionnaire. Answer questions about your household income level and monthly expenses. Click the "Start here" tab to begin.

Request an OHP application packet. Contact 800-359-9517 or see Resources for a link to an electronic version (in PDF format) of the OHP application packet you can download.

Contact 800-282-8096 if you currently receive Social Security Insurance or are at least 65. Ask if you qualify for OHP or other Oregon health programs.

Bring copies of your most recent paystubs, your medical records (if you are pregnant), any other insurance documents that you have and a copy of your student aid report (if applicable). If you and your family qualify for coverage, bring copies--not originals--of your required documents.
