What to Bring to Apply for Medicaid

Medicaid is a government program that offers health care to those who cannot otherwise afford it. Funding for this program comes from both the federal and state governments, but each state manages the program within its boundaries. Thus, there are different specific requirements for each state, but most of the documentation you need to bring to apply for Medicaid is necessary in all states.
  1. Proof of Income

    • Medicaid is a needs-based program that is only eligible to people who meet certain income requirements. You must show that your income is less than the limit with proof of your income. This could include pay stubs or statements from your other income sources, like Social Security or child support.

    Proof of Citizenship or Alien Status

    • Except in cases of pregnancy or needing emergency treatment, Medicaid is only available to U.S. citizens or resident aliens. You must show proof of your citizenship, such as a birth certificate or passport, or proof of your alien status, such as your green card.

    Application

    • You must fill out an application for Medicaid. You can receive this from your state's department of health. Many will allow you to download the application from the website. Fill out this form fully before going to apply to save yourself time.

    Other State-Mandated Items

    • Since each state has specific requirements for Medicaid, you should double check to see if there are additional things that you must bring. In Florida, for example, you must document all of your property and list all cars that you own.
