Iowa Medicaid is federally funded health insurance that provides health care to low-income residents. The Iowa Department of Human Services (DHS) manages the Medicaid program. Gather your documents. Before beginning the application process, you will need each household member's name, date of birth, Social Security number, current income and resources. Make copies of pay stubs and birth certificates to submit with your application.

Go to the Iowa Department of Human Services website to complete your application. Iowa Medicaid is available to children under the age of 21, parents or guardians living with children under the age of 18, pregnant women, women with breast or cervical cancer, blind, disabled or elderly.

Meet the criteria to apply. You will need to be a U.S. citizen and reside in Iowa.

Answer household questions. You will need to list all household members, their relationship to you, their relationship to each other, and any absent parents. You must provide all sources of income and assets, including your bank accounts and vehicles, and list monthly expenses. In 2011, the monthly income for Medicaid coverage for an infant is 200 percent of the federal poverty level, which is $2,767 for a family of three.
