How to Request a Medicaid Confirmation in Iowa

Iowa Medicaid is federally funded health insurance that provides health care to low-income residents. The Iowa Department of Human Services (DHS) manages the Medicaid program. Along with your application, you will need to submit documents, including your driver's license, Social Security card and birth certificates for household members.   Gather your documents. Before beginning the application process, you will need each household member's name, date of birth, Social Security number, current income and resources. Make copies of pay stubs and birth certificates to submit with your application.

  Go to the Iowa Department of Human Services website to complete your application. Iowa Medicaid is available to children under the age of 21, parents or guardians living with children under the age of 18, pregnant women, women with breast or cervical cancer, blind, disabled or elderly.

  Meet the criteria to apply. You will need to be a U.S. citizen and reside in Iowa.

In 2011, the monthly income for Medicaid coverage for an infant is 200 percent of the federal poverty level, which is $2,767 for a family of three.
