Coventry's provider network consists of primary care providers, doctors, facilities, hospitals and other health professionals. Providers can join the Coventry provider network by contacting the provider services department at 1-800-937-6824. Alternatively, to complete an application online, visit the Coventry website. Input information such as provider name, contact information, specialty, tax identification number, national provider identification number and hospital affiliations, among other information. It may take several weeks to process the application but you can check the status by calling provider services for an update.
