1
Gather your information. You'll be required to supply proof of your income or lack of income, how much money is in your checking or savings accounts, the annual cost of your health care (including current prescription costs) and personal information, such as your Social Security number and home address.

2
Using Microsoft Word or another word processing program, select a professional template in which to write your letter. The letter should include your mailing address at top, the date and the address of your state representative.
-
3
Write your letter, addressing it to each state representative by name. Be as professional and as concise as possible. In one paragraph, explain why you are requesting help (perhaps you have already been denied for Medicaid) and include information about your circumstances, including how much income is currently available to you. If you require funds for health care, explain your health care needs in detail. In your closing paragraph, include a statement letting your representative know that you are willing to send copies of your bank statements and doctor's bills upon request to support your need. Do not send this information unless it is requested. In closing, thank your representative for his time and consideration, and include your phone number in case more information is needed.

4
Mail your letter. Most state representatives have websites that include contact information.
