How to File for Disability in Oklahoma

According to the Oklahoma Department of Rehabilitation Services, one in six Oklahomans over the age of 5 has a disability. About 14.9 percent of work-age Oklahomans are disabled in some way and need government assistance to provide for their families. If you've become injured physically or mentally to the point where you cannot work, you may be eligible to get social security disability benefits.   1. In-Person or Phone Appointment

      Find your nearest Social Security Office. The Oklahoma Department of Rehabilitation Services provides a list of Social Security offices in the state on their website (See Resources). There are 19 Social Security offices throughout the state. The listing will provide the address and phone numbers for each office.

      Call 800-772-1213 to be connected to your nearest Social Security office in Oklahoma for an appointment. You can also use the Social Security Office Locator tool on the Social Security Administration's website to find the local number to an Oklahoma office nearest to you (see resources). Request an appointment to apply for disability benefits. You can request an in-person interview or an over-the-phone appointment. Social Security offices in Oklahoma are open Monday through Friday from 9 a.m. to 4 p.m., except on federal holidays when they are closed.

      Provide necessary information at the appointment. Whether you are speaking with a claims agent over the phone or in-person, he will fill out everything for you as long as you provide him with the information. You will need last year's W-2 Form, Social Security numbers for you, your spouse and your children, any military discharge information, bank account number, work history, any worker's compensation claims you may have filed, and full and complete medical records and all prescriptions. The interview should take about an hour. You will be asked to sign all forms before your application is sent for processing.

      Go to www.socialsecurity.gov, the official U.S. Social Security Administration website. Click on the "Disability" tab at the top of the page. Be prepared to enter all of the same information as above, such as work history, W-2, Social Security numbers and medical records.

      Click the "Apply for Disability Benefits" link. On the resulting page, click the "Apply Online for Disability Benefits Box," a red box on the right side of the screen. This will take you to the first part of the application, the Online Disability Benefit Application. Complete the application with the necessary information to the best of your ability. You will need to provide all information regarding your income and you family's financial situation overall, as well as taxes you have paid.

      Complete an online Adult Disability Report. After you finish with the application, click on the link for "Adult Disability Report" and provide information to complete the report. This is where your medical records will come in handy. The report will ask you questions about your medical history, the doctors who have been caring for you, and the prescriptions you have been taking. You will also need to provide information on your work history and educational background.

      Download and print the Authorization to Disclose Information to the Social Security Administration form. Complete and sign the form and submit it at your nearest Social Security office in Oklahoma.
