Seven Steps to Obtain Medicaid in Michigan

Michigan Medicaid helps people with low income, including children, elderly people, people with disabilities and pregnant women, pay for health care services. It covers things like hospitalization, outpatient visits to doctors and clinics, mental health care, some dental care and prescription medications. The Department of Community Health administers the Medicaid program in Michigan and you must apply in the county in which you live.
  1. Obtain an Application

    • Obtain an application for Medicaid from the Michigan Department of Community Health in the county in which you live. If you prefer, complete an application online (see link in Resources section).

    Complete the Application

    • Complete the application. Provide information about all people who live in your house, even if they do not want to apply for Medicaid. Do not leave out any information or the Department of Community Health may be unable to process your application for assistance.

    Turn in the Application

    • Turn in the application at the Michigan Department of Community Health in the county in which you live. You can take it there in person or mail it. Submit your application online if you prefer.

    Provide Requested Documentation

    • Provide the requested documentation, like proof of household income including money earned from employment and disability or Social Security payments, proof of any assets or savings, proof of living expenses, proof of pregnancy or disability and information about any health insurance policies you have. The Michigan Department of Community Health may ask you for additional documentation based on your circumstances.

    Request Assistance if Needed

    • If you need help completing the application or getting any of the requested documents, ask for help at the Michigan Department of Community Health office in the county in which you live. Staff there will assist you.

    Attend an Interview

    • Attend an interview at the Michigan Department of Community Health office at the scheduled time. If you cannot keep your appointment for any reason, call the office and ask to reschedule. If you cannot go to the office because of a disability, call the office and ask if you can be interviewed over the phone or if a caseworker can visit you at home.

Notify Your Caseworker of Any Changes

    • Any changes in the number of people living in your household or in your household income may affect your eligibility for Medicaid in Michigan. Notify your caseworker at the Michigan Department of Community Health of any changes as soon as you can.
