Ensure that you meet the income eligibility requirements. The income limit is $217 per month per individual in the household. The resource (assets) limit is $2000. For each additional person living in your household, add $70 to the income limit.

Visit your local Department For Community Based Services (DCBS). If you need to find an address for your nearest DCBS office, use the "Local Office Search" tool that is located on the Kentucky Cabinet For Health & Family Services website.

Request an application for Kentucky Medicaid. Complete the Medicaid application.

Present the completed application to the DCBS representative. Along with the application, submit a copy of your Social Security card, driver's license or state issued photo ID, birth certificate and proof of income, bank statements and medical records to prove pregnancy (if applicable). Proof of income can include check stubs, tax returns and 1099 forms.
