How to Request a New Medicaid Card in Michigan

Michigan's plastic Medicaid card, also known as the MiHealth Card, is considered a permanent card. It should never be thrown away, even after Medicaid benefits expire. If your card becomes lost, damaged or stolen, there are methods in place for you to replace it. If your name or address has recently changed, it is necessary to contact your Department of Human Services case worker. The case worker will update the database with your new information. Once you are certain that all of your database information is correct, proceed to request your new card.

      Contact the Beneficiary Help Line at 800-642-3195.

      Advise the help line representative that you would like to request a new MiHealth Card. Advise the representative of the reason you are requesting a new card. For instance, if your old one has been lost, damaged or stolen, inform the representative.

      Provide information to the help line representative to confirm your identity. The representative will put in a request to have a new Medicaid card mailed to you. It will take approximately one week for you to receive your new card in the mail.
