I Lost My Medicaid Card, How Do I Get a New One?
While Medicaid is a federally funded program, it is actually administered at the state level. That means that if you have coverage questions, or if you need to replace a lost or stolen Medicaid card, you need to contact the office of your state Medicaid coordinator to request a new card. Every state handles its own Medicaid card replacement. You may be able to get emergency care in the meantime, as care providers cannot turn you away for bona fide emergencies.Instructions

Click on the link given to the National Association of State Medicaid Directors given in Resources.

Locate the menu located on the left-hand side of the screen, in the green field. There are a series of buttons you can select.

Click on the button marked "NASMD Home." It will open a sub-menu. Select "Members." This will enable you to download a file listing your state's Medicaid director offices.

Look up your state on the list of state Medicaid directors.

Gather any information on Medicaid recipients you have. You will need to provide full names, birthdays and Social Security Numbers.

Call the office of the state Medicaid director. The receptionist or operator will direct you to the appropriate extension for your state.

Request the new card. Then look for the envelope to arrive in the mail. Some states may be able to email you a copy of your lost card.
