Aetna's Preferred Provider Organization (PPO) plan offers customers access to a preferred provider network, which includes hospitals, specialists and family doctors. Referrals are not required with this plan, nor is the selection of a primary care physician. The plan has two coverage levels. The first provides a set of benefits for visits to network providers, and the second allows for a set of benefits for care provided outside of the network. The health plan pays more for visits inside the network. Coverage includes annual physicals and exams, specialist care, outpatient surgery, prescription drugs, emergency care and hospital visits.
Managed Choice Plan

Combining aspects of health maintenance organization (HMO) and PPO plans, Aetna's Managed Choice plan lets customers pick which option they use every time they get health care. Customers may visit any health care provider, though those who select an in-network primary care physician to oversee their health care will pay less. If you choose to visit an out-of-network provider, you will pay a larger share of the overall costs. The plan covers office visits to primary care physicians and specialists, prescription drugs, lab work, x-rays and annual physicals.
