How to Apply for New Jersey Blue Cross Blue Shield Provider Contracts

Horizon is the only independent of licensee of Blue Cross Blue Shield in New Jersey, administering HMO plans, PPO plans, dental plans and more. Horizon is New Jersey's largest health insurer with over three million members throughout the state. Contracting with Blue Cross Blue Shield of New Jersey gives providers the opportunity to see more patients in their area and be reimbursed at contracted rates for their services.

      Contact Horizon Blue Cross Blue Shield of New Jersey and speak to a recruitment representative. Phone numbers for recruitment representatives are listed on their website.

      Ask your recruitment representative for information on the contracting and credentialing process. Download the provider credentialing and re-credentialing policy from Horizon's website as a brief overview of the process.

      Complete your enrollment application. You will need a National Provider Identification (NPI) number, medical license, Drug Enforcement Agency (DEA) or controlled substances certificate, and proof of liability coverage. You may be asked to provide copies of all of these documents.

      Sign and date your application. Refer to Horizon's credentialing policy before submitting your application. Be sure you have included and attached all required documentation.

      Send the application to Horizon Blue Cross Blue Shield of New Jersey. According to Horizon's credentialing policy, providers must return the credentialing application within 60 days of receiving it.

      Wait for the credentialing process to be completed. The application and supporting documentation is verified and then forwarded for approval by a credentialing committee. It is Horizon's policy to review and approve applications within 90 days of receipt.

      Sign your reviewed contract agreement once you are accepted as a participating provider.
