Fill out the form by including accurate information about your disability, when the disability began, and the date you were first absent from work. You will also need to provide your contact information, social security number and details about any other health claims you have made concerning your disability, such as workers' compensation.

3
Get your employer to fill out his portion of the form, which outlines how much you earn, how many hours you contribute per week, the specifics of your disability in relation to your work, and the first day of work you missed.

4
Get a physician to validate the information you have filled out on the form. The physician will have to disclose information about your treatment history, the diagnosis of your disability and other symptoms.

5
Send the completed form to your insurance provider. Your provider may request access to your medical records to complete an audit. It usually takes up to 15 days to process the form, but once you have been approved, you will immediately receive compensation for the duration of the short-term disability.
