How to Obtain a Health Insurance License in Arizona
To sell health insurance in Arizona, you must first obtain an Accident and Health or Sickness License from the Arizona Department of Insurance (ADI). Applicants must be at least 18, pass a licensing examination and submit to LiveScan fingerprinting. You must renew your initial license after three years, and then every four years thereafter. The ADI has contracted with a private company, Prometric, for administration of the examination process.Instructions

Register for the Accident and Health or Sickness License examination with Prometric. You can do this on the Prometric website or by calling 800-853-5448. You will have to pay a $49 examination fee by credit or debit card.

Pass the licensing examination at one of Prometric's testing centers in Arizona. The examination is computer-based and consists solely of multiple-choice questions. You must score at least 70% to pass and you will receive your grade upon completion of the examination.

Have your fingerprints taken by a fingerprint technician at the testing center.

Submit your licensing application to the ADI. You can do this at a computer kiosk located at the testing center or by downloading an application from the ADI website and mailing it to:

Pay the $120 license fee by debit or credit card if you are applying online or by check or money order if applying by mail. Your license will be mailed to you upon approval by the ADI.
