How to Get MassHealth Insurance

Massachusetts offers a variety of health insurance coverage plans under its MassHealth program. This insurance program is structured for low- and medium-income people. MassHealth also considers other factors for eligibility, such as age, family structure, whether a family member is pregnant, and whether you are in a long-term care facility. To find out whether you are eligible for MassHealth, complete the application.

      Go to the Health and Human Services website of the Massachusetts government. Click on the orange "For Consumers" tab at the top of the page.

      Scan through the list of services until you see one labeled "Insurance (Including MassHealth)." Under this heading, click on "Apply for MassHealth."

      Review the list on the next web page, and click on the link that best describes you. For example, there is a link for applicants older than 65; for applicants under 65 (and families), and for people who require long-term care.

      Read the eligibility guidelines. If you find you are eligible, click on "How to Apply," or "The Application Process." The exact wording of the correct link will vary, depending on which section of the website you selected in the previous step.

      Download the Medical Benefit Request (MBR) form. Fill it out. You may either fill it out on your computer and print it, or you may print it out and write on it by hand. The form requests basic information, such as your full name and names of family members; dates of birth; Social Security numbers; income and employer information; and the information for any current health insurance you have. The form also collects information on disabilities and illnesses. Sign and date it after you finish filling it out.

      Gather relevant documents. MassHealth requires proof of all income (except SSI or social security); proof of identity (such as a copy of a birth certificate); copies of immigration cards, if applicable; and proof of HIV-positive status, if applicable.

      Mail the application and relevant documents to the MassHealth Enrollment Center. The address is on Page 3 of the application.

      Wait for written notice of acceptance. If MassHealth requires more information or clarification, they will contact you before making a decision.
