1
Obtain a medical assessment to verify your need for a wheelchair. Some insurance companies and Medicare and Medicaid plans may require you to be assessed by one of their company appointed doctors.
-
2
Receive a prescription or Detailed Written Order (DWO) for a wheelchair from the doctor who assessed your condition.
-
3
Order your wheelchair from a medical supply dealer. Some medical supply dealers may be able to fax your information to and bill your insurance company. Ask for details when ordering.
-
4
Fax your order receipt to the insurance company, along with a copy of your prescription and personal details, including your full legal name, address, phone number, the fax number and UPIN (Unique Physician Identification Number) of the doctor who prescribed the wheelchair. Include the prescription and a detailed description of your condition, your birth date, weight, height and your insurance policy number.
