The five plans vary primarily by co-payment for doctor visits that range from $15 to $50 to see your primary-care physician as of September 2010. The program also allows you to see specialists for a co-pay between $15 and $70. A coinsurance plan is also available.

    Aetna

    • Aetna's plan includes coverage for checkups and preventative care, specialty care, hospitalization, surgery and emergency care as well as mental health and substance-abuse programs. The plan does not cover cosmetic surgery, hearing aids, custodial care, travel immunizations or any services not recommended by your primary-care provider. Co-pays for office visits range from $15 to $30 as of September 2010 depending on the level of care you select.

    AmeriHealth

    • The AmeriHealth company offers HMO coverage for individuals through its AmeriHealth HMO programs. The program participates in a network of providers in New Jersey, Delaware and Pennsylvania. Members of the plan pay between $15 and $50 for a visit to their primary-care provider as of September 2010. Depending upon your level of coverage you may also be eligible for X-rays and lab diagnostics at no additional fee. Additional benefits include mental health care, substance abuse treatment and a prescription drug plan. While benefits vary depending upon your level of coverage, there is no lifetime maximum on coverage.
