How to Renew Med-QUEST Health Insurance

If you are a Hawaiian resident and you meet the eligibility criteria, you may be able to receive comprehensive low-cost medical care through the Med-QUEST program. This is a state paid health plan for those under 65 years of age who are not blind or disabled. If you are already receiving Med-QUEST benefits, you need to ensure that you currently meet the income and qualification criteria when renewing your medical coverage.

  Check your Med-QUEST card for the expiration date of your current benefits. Be sure to apply prior to that date.

  Go to the Med-QUEST website and download a renewal form (see the Resources section). Review the form carefully and gather all of the documentation that you will need to complete it.

  Enter your information and the names of all household members. Also list everyone who does not currently have health insurance. Provide all of the requested information about each person in the household, including the dates of moving in or out of the home.

  Provide the income information requested on the application. Be sure to include the source of the income as well as the name of the person receiving it. Consult your tax returns, pay stubs and other financial records to gather this information.

  Enter information about your childcare arrangements in the applicable part of the form. Include the name of the person or agency paying for the childcare expenses, as well as the amount.

  List your household assets in the assets section of the Med-QUEST renewal form. Enter a detailed description of each asset, the name of the person who owns it and the cash value of the property. Examples of reportable assets include homes, cars and bank accounts.

  Enter information regarding any medical insurance covering those in the household. Provide details about the policy, including the effective dates.

  Provide information about any household members who receives care in a nursing home. Detail the assets that were sold to pay for that care.

  Read the declarations on the renewal form carefully, then sign it. If someone helped you complete the application, that person must sign and date it as well.

  Submit the Med-QUEST renewal application to one of the addresses listed at the bottom of the form. You may drop it off in person, mail it or fax it to your local office.
