Contact the company that serviced the previous health-care coverage that was provided by the employer. The insurance company is required to contact the newly qualified person of their eligibility for COBRA coverage within 14 days of being notified by the employer that the group-health-insurance coverage has ended.

Request the forms. Usually the insurance provider will automatically provide the forms that are needed to select COBRA coverage. In case they were not sent, however, make the phone call and request them. COBRA insurance can't be elected without the right forms.

Fill out the forms and provide the required documentation from the previous private insurance provider. Mail the forms and necessary payment back to the COBRA office. Confirmation and a policy identification number will be mailed to you.
