How to Find the Expiration Date for a BCBS PPO Health Insurance Policy
Blue Cross Blue Shield (BCBS) offers personal and employee health insurance all over the United States. Each state has separate “hubs” to handle the state’s insurance policies for BCBS. If you feel that your plan is about to expire, you need to find out exactly when so you know not to schedule any doctor appointments when you're not covered. Check your card. Some BCBS cards list the active and expiration date on the card. If your card does not list an expiration date, go to step two.

Call the customer service phone number on the back of the card.

Follow the phone prompts to be forwarded to a customer service representative; you can also try pressing “0” on your phone or saying “customer service” into the phone. Each state will handle the phone service differently. Even if you can obtain eligibility information through the automated system, the expiration date may or may not be listed. It is best to just speak with a representative.

Speak with the representative. The representative will ask you to verify some personal information such as your date of birth and Social Security number before giving you any information on the account. After the information is verified, the representative will be able to inform you when your plan expires.
