With more than 1,400 hospitals and clinics in the United States, there are many resources available to eligible U.S. military veterans to receive Veterans Administration medical benefits. Whether it's simply immunizations, physical examinations or something like emergency room care, the VA is there to help its veterans. There are different criteria which the VA uses to determine if a veteran qualifies for VA medical benefits, but initially the veteran needs to apply for medical benefits using either of the VA application options.Instructions

Assemble the following documents before beginning the application for VA medical benefits: Paperwork related to your separation from military service (DD214, medical discharge paperwork); any current health insurance information; and previous year's household income. Part of your qualification for VA medical benefits can be based on your household income.

Fill out a VA Form 10-10EZ - Application for Medical Benefits. You can either fill out the VA Form 10-10EZ - Application for Medical Benefits by completing the form online, or you can print out the form and mail it/take it to your nearest VA facility. If you would like, a VA Form 10-10EZ - Application for Medical Benefits can be mailed to you. Call the VA at 877-222-8387 Monday to Friday, 8 a.m. to 8 p.m. to request a copy be sent to you.

Mail your completed VA Form 10-10EZ - Application for Medical Benefits if you did not use the online feature to your nearest VA facility. If you are unsure where your nearest VA facility is, visit the VA's website to help you find the nearest facility by entering your zip code.
