Meet eligibility requirements. Apply only if your children are age 18 or under and are U.S. citizens, qualify as legal residents, or are official refugees. Submit a U.S. birth certificate, a U.S. passport, certificate of naturalization, or a certificate of U.S. citizenship to prove eligibility. According to peachcare.org, if you send one of these, no other citizenship eligibility information will be required.

Satisfy a six-month uninsured provision. Eligible children must be uninsured for the six months prior to applying for Georgia Peachcare. There are exceptions for children who have lost coverage involuntarily during that six-month period because of a parent who has lost a job or has been unable to work. Also, according to peachcare.org, the waiting period does not apply to families who had independently purchased private insurance outside of an employer group.

Apply for Georgia Peachcare. Visit a local Georgia Department of Medical Assistance to complete the application process. Research locations at peachcare.org for the facility nearest you. Alternatively, apply at peachcare.org.

Choose a Georgia Families Care Management Organization to coordinate your child's health care. A list of available coordinators will be available to you when you apply in person or submit an application via peachcare.org. You will also be able to choose your child's primary care provider.

Receive Georgia Peachcare insurance once eligibility requirements are met. As stated on peachcare.org, family income must be less than or equal to 235% of the federal poverty level.
Proof of family income is required to complete the application process and receive insurance. Four weeks of weekly pay stubs, two semi-monthly pay stubs or two monthly pay stubs are required. Proof of money anyone in the household receives from any agencies, relatives, friends, or any other sources must also be submitted.
