How to Apply for NY's Medicaid

Medicaid is a United States health program for those with low income and resources. New Yorkers unable to afford private health care options may qualify for Medicaid. New York residents may get an application for Medicaid by visiting, writing or calling their local Department of Social Services, or calling the Human Resources Department.

      Meet the income requirements for medicaid. If your income exceeds the income limits, you will not be able to qualify. The income requirement varies depending upon how many children you have, whether or not your pregnant, and if you have a disability. To verify that you meet income requirements see Resources.

      Get an Application for Medicaid and fill it out. You must obtain this application in person, by writing your local Department of Social Services, or calling. To obtain the contact information for your local social services division see Resources. You may also contact the Human Resources Department at: 718-557-1399 and request an application by mail. Pregnant women and children may apply for Medicaid at local hospitals and clinics. You must call your local Department of Social Services to find out at what hospitals and clinics you may apply at. After the application is complete, you may send it in or return it in person to your local social services office.

      Collect all necessary documentation for your Medicaid interview. The local Department of Social Services will schedule an interview for Medicaid with you once they receive your application. It is important to make sure you have all necessary documentation to prove your identity and income, such as a birth certificate to prove your age; proof of citizenship or alien status; recent paycheck stubs if you are currently employed; proof of other forms of income, such as Social Security, child support or retirement; proof of residence, such as a rental agreement or current insurance benefit card.
