How to Get Services from Health Net PPO

Health Net is an online company offering health- and dental-care insurance plans. The plan known as PPO, or preferred provider organization, is one style of the company’s health insurance plans. In a PPO plan, the patient selects a single doctor for primary care. The provider focuses on coverage for residents of California, Arizona, Oregon and Washington states. For residents of other states, the Health Net system will simply refer users to another company, called eHealth Insurance, for a quote or application via its website. All PPO plans through Health Net are underwritten by Health Net Life Insurance Company.

  To enroll in a PPO plan with Health Net, go to the website healthnet.com. This will direct you to a main home page for the insurance company. A blue box at right will display several options. Click on the “Enroll Now” box to begin applying for a plan.

  Click on the green-colored state name which coincides with your area of residence. If you are not a resident of one of the four states, click on “All other states” to be directed to the eHealth Insurance provider, an alternative insurance company.

  After selecting your state, a form for application will display on the screen. Fill out the blank fields, starting with your zip code and county of residence. Next, you will be asked to select from a drop-down menu whether you would like to apply for long- or short-term coverage and to pick a date for coverage start. Next, enter your date of birth and gender, along with the same information for any dependents you may have.

  Click on continue when you have finished the first form. You will then be directed to a page to select a plan that fits your needs. Click on the box beside a plan with the letters “PPO” included to select this style of health care coverage. For example, the Optimum Advantage HSA 4500, 2500 or Value Net plans are all types of PPO. You will see “(PPO)” in parenthesis to the right of the plan’s name if it is a PPO plan

  Submit your request for enrollment by clicking “Apply Now.” Complete the subsequent forms, which request your personal information such as name, social security number, address, contact telephone number. These are secure forms. You should see a small lock on the bottom right of your Internet browser display. This means others will not be able to read what you type. All blank fields must be completed, or the form will not continue. Click on “Next” when complete.

  Upon completion of the remaining application forms, a representative from Health Net will review your information and be in touch with you.
