How to Switch From Tricare Prime to Standard
Tricare Prime is a comprehensive and affordable medical insurance plan for active duty military, military retirees and their family members. Tricare Standard is a fee-for-service insurance plan offered through Tricare; it has higher out-of-pocket expenses for deductibles and copayments than Tricare Prime, but the insured can be more selective in their choice of medical providers. Tricare Standard policyholders may see either in-network or out-of-network providers for medical services. Switching from Tricare Prime to Standard is only an application form away; once you complete the necessary paperwork, the switch from Tricare Prime to Standard takes a matter of days.Things You'll Need

How to switch from Tricare Prime to Standard

Use a black ink pen to complete the Prime Disenrollment form. The information you will need includes your sponsor's Social Security number, the date of birth of sponsor, your relationship to the sponsor, your mailing address and a contact phone number.

Review to the form to ensure that all the information you entered on the Prime Disenrollment form matches the information on file with the Defense Enrollment Eligibility Reporting System (DEERS). The Prime Disenrollment form is available at www.humana-military.com.

Mail the form to the address provided on the form. If your Tricare area contractor has a different address from the return mailing address provided, contact the Tricare Regional information line at 800-874-2273 or visit www.tricare.osd.mil to obtain an accurate address before mailing the Prime Disenrollment form.

Allow seven to 10 business days for your Prime Disenrollment form to be received and processed by Tricare.

Call Tricare to confirm receipt of your Prime Disenrollment form at 800-444-5445; the representative will confirm your new status. You will not need to fill out a form for Tricare Standard. You have now switched from Tricare Prime to Standard and will receive a Tricare Standard handbook covering pertinent information regarding your benefits and eligibility for medical services.
