How to Get ODS Health Insurance

With medical plans, dental plans, Medicare plans, and a Rx discount card program, ODS Health Insurance offers affordable health insurance to individuals and families, employers and agents. Prospective members and employees have several enrollment options. Before you purchase or enroll in ODS Health Insurance there are a few important facts you should know about ODS and its many plans. Note that ODS Health Insurance is only available to Oregon residents.

  Contact your company's health insurance plan administrator and ask if your employer has purchased group health insurance through ODS Health Insurance. ODS Health Insurance offers a comprehensive small business health insurance plan. The plan is eligible to all compensated owners, partners and permanent employees who work a minimum 25-hour work week. Seasonal employees are excluded from participating, however all other employees receiving salary and drawing a W-2 are eligible.

  Visit the official ODS website to obtain a quote and apply online (see Resources). View plan rates and benefits to select the best plan for you and your dependents and/or spouse. All rates are subject to the number of insured dependents as well as the age of the insured. Rates are listed based on the following categories: insured only; insured and spouse; insured, spouse and children; and insured and children. Apply online or request a paper application by mail. Application requires insured and dependent information, including plan effective date, age of insured, and date of birth for each dependent.

  Contact an ODS agent in your area or contact an ODS Health Insurance Individual Plan Sales Representative or ODS Health Insurance Marketing specialist at [phone] for individual or group plans. Group and Individual agents are available to answer questions and assist you in your purchase of ODS Health Insurance. Complete an application and make the first monthly payment to begin insurance coverage. Perform a provider search to verify that your OB/GYN or family physician is within the ODS provider network.   Print a PDF paper application (see Resources below). Complete the 8-page document in its entirety, sign and date and then mail to: The ODS Companies | Attn.: Individual Underwriting | 601 S.W. Second Ave. | Portland, OR 97204-3156.

  Wait for enrollment period to begin. Applications are due no later than the 20th of the month, wherein the application is successfully accepted to begin insurance coverage on the first of the following month.
