Ask if your doctor or medical facility accepts your particular insurance before obtaining services from them. Chances are your claim may be denied or not paid at the higher levels should you seek medical attention from a non-participating provider.

Keep receipts, itemized bills and other documents to attach to your health insurance claim. Make sure the bill lists all services the doctor performed with adequate insurance codes and costs for each service.

Fill out a health insurance claim form. Get a copy of this form from your insurance company. You may be able to download and print these forms from your health insurance company's website, or by calling a toll-free number and having them mailed to you.

Make any necessary payments to the medical facility you are visiting and include required paperwork to get reimbursed, if necessary. Some health insurance companies require the patient to pay up front and get reimbursed later.
