How to View My Disability Report
The adult disability report is one of the most important items you will need to submit to receive disability benefits through the Social Security Administration. The report asks questions about your current employment, your medical condition, and how this condition prevents you from working or interferes with your daily life. You can complete the report online on the Social Security Administration website (ssa.gov), and it is important to have a copy for your records.Instructions

Complete the disability report online. When you are finished, you have the option to view and print the final copy. The Social Security Administration recommends that you print a copy of the report and the receipt for your records. (See Reference 1.)

If you forget to print your report, you may write to the Social Security Administration and request that a copy is mailed to you or visit your local branch of the Social Security office in person and request copies of your forms. The clerk may be able to print a copy immediately or you can have a copy mailed to you. (See Reference 3.)

You can write to your local Social Security office if you cannot visit in person and request a copy of your completed Adult Disability Report. Include your Social Security number, name, and address when you write so the recipient can easily identify you and find your records.
