How to File SSI for Vertigo
Vertigo, which causes extreme dizziness and disorientation, can make working difficult if the condition is chronic. This disorder is often caused by Meniere's disease or an inner ear infection. If this is the case, the sufferer may apply for SSI (Supplemental Security Income) if he will be disabled and unable to work for 12 months or longer.Instructions

1
Determine your eligibility for SSI. To be eligible, you must reside in one of the 50 states, District of Columbia or the Mariana Islands (you are not eligible if you leave the country for more than 30 days), and you must be a U.S. citizen or resident alien. The vertigo must also last for at least 12 months.

2
Take the SSI eligibility benefits test on the Social Security website (see Resources 1). This can help you determine if you can proceed with your claim.

3
Gather documents to send in with your application. These include your Social Security card, identification with proof of age such as a birth certificate or religious record, proof of citizenship or resident alien status, proof of income in the form of payment stubs or tax returns, proof of resources which may include bank statements, deed or tax appraisal statements, insurance policies and titles for vehicles. You will also need to provide a detailed work history including the name of your employer, dates of your employment, how many hours worked per day and week, pay rate and a description of the job you did. In addition, you will need to provide medical records for your vertigo, contact information for your doctors and a list of medications.

4
File an application at your nearest Social Security office for your disability due to your vertigo. Be prepared to show a statement from your doctor saying that your vertigo inhibits you from working.
-
5
Copy all of your documents before sending them out or giving them to the Social Security office.
