Veterans who receive care at a Veterans Affairs hospital may have to fill out a Medical Expense Report form in order to remain eligible for care or to reduce co-payments. All payments for medical care for you or your family should be listed on this; these payments will be deducted from your income as determined by VA. If this form is required for you, it must be submitted to VA once a year.

      Get the a copy of the Medical Expense Report Form from your local VA.

      Fill out your personal information on the form.

      Contact all medical providers that you or your family have used in the last year. This includes family doctors, eye doctor, dental work or emergency room visits. Any healthcare that you or your family had in the last year may be counted on your Medical Expense Report if you paid for those expenses out of pocket.

      Contact your insurance provider if applicable. Any money that you have paid for the insurance policy, co-pays, deductibles and premiums may be counted on the report.

      Request an itemized list of all visits by you and your family to all of your medical providers in the last year. This will provide you with an easy guide to all of your expenses for the time period.

      Fill out Part Five of your Medical Expense Report with this information in the boxes provided. Be sure to include all of the information that you have received.

      Sign and date the form.

      Deliver the form to the Intake Office at your local VA Hospital. This will greatly expedite the process, as the form does not have to go through both the Postal Service and Veterans Affairs systems.
