Have the doctor or a nurse practitioner fill out the lab request, as they will be aware of what tests and lab test you need done to better diagnose your chiropractic problem. It could range from simple blood work, to x-rays, to an MRI scanner that examines muscular and joint connections between limbs.

Ensure that the doctor has signed all of the appropriate boxes, then fill in your signature under any fields labeled "patient signature" so that the laboratory knows that they have both your and your doctor's consent.

Bring the completed order form to your local lab or clinic that will be performing the test. Explain to the receptionist that if they have any questions, they should contact the doctor on the request form, and that any and all test results will be sent to that doctor's office (or sent to you, and you will bring them to your chiropractor yourself).
