New Jersey SNF Medicaid Documentation Requirements

When one falls ill, the hospital is not necessarily the most appropriate place to receive care. It is sometimes advised that the patient be transferred to a skilled nursing facility (SNF). The state of New Jersey offers coverage for SNFs to candidates for such care who have submitted the required documentation, and are deemed eligible to receive Medicaid. Such documentation should be gathered and submitted to the appropriate county Board of Social Services.

Identity

• Submit documentation that will provide proof of age, citizenship, marital status and identity. Accepted forms include state issued identification card, driver's license, passport, birth certificate or military identification. It is also necessary to submit any copies of marriage or death certificates, as well as divorce decrees or separation papers.

Resources

• Medicaid recipients must also submit proof of any available resources, including checking, savings and money market accounts, as well as retirement accounts, stocks and bonds. It is also necessary to provide any statements pertaining to credit union shares and life insurance policies with cash value.

Living Expenses

• Medicaid recipients wishing to enter a skilled nursing facility must also submit documentation of monthly living expenses. Such expenses may include electricity, telephone, water and sewer bills, rent, mortgage, health and homeowner's insurance.

Finances

• Medicaid is awarded on the basis of financial need and, as such, it is necessary to submit documentation of monthly income. Acceptable forms of proof include copies of recent pay stubs, social security and disability checks, as well as worker's compensation, unemployment and alimony checks.
