How to Transfer Medical Assistance From MD to Pennsylvania
If you receive Medicaid and plan to move from Maryland to Pennsylvania, you'll need to transfer your medical assistance. Do this as soon as you move or right before you move so that you do not have a long lapse in medical coverage. Contact the Maryland Department of Health and Mental Hygiene office in the county in which you lived to inform them that you have moved or will be moving to Pennsylvania. Let them know the exact date that you will become a Pennsylvania resident.

Contact the Pennsylvania Department of Public Welfare office in the county in which you now live to request an application for Medicaid. If you prefer, you can download an application online or apply online (see Resources).

Complete the application and take it to the Pennsylvania Department of Public Welfare office in the county in which you live or submit your application online. If you need help completing your application someone at the Department of Public Welfare office will assist you.

Provide verification of your income (such as paycheck stubs or a disability award letter), resources (such as bank statements), medical bills and any other information requested by the Pennsylvania Department of Public Welfare.
