How Do I Change a West Virginia Medicaid Card?
If you live in West Virginia and are enrolled in Medicaid, you receive a monthly Medicaid card verifying current eligibility. If you have selected a Managed Care Organization (MCO) and primary care physician (PCP) through enrollment in Managed Care, you should have also received an insurance card from the MCO. Both cards are required as proof of insurance coverage to receive medical care or prescriptions. Visit the West Virginia Department of Health and Human Services website at wvdhhr.org to report a change of address. Select "Report changes online "from the Online Services menu and click "Get Started." Check the box next to "Change of Address" and provide requested information. Complete the Change Request Form and click "Submit My Changes" to send request and enact changes to your Medicaid card.

Call 800-449-8466 to request a change in your Managed Care Organization (MCO) or health plan provider. You may choose to keep the same physician if he accepts the new plan. Confirm that a new insurance card reflecting the recent changes will be sent by mail and validate your current address.

Contact the HMO at the toll free Customer Service number listed on your insurance card to change your PCP to a different physician within the provider network. Be sure to verify that the new doctor is accepting new patients. Confirm the address on file and request an updated insurance card.
