How to Obtain Free Health Care in Tuscaloosa, Alabama

Alabama Medicaid provides health care to eligible residents with limited income and resources. Those covered include pregnant women, families, children under 19, women ages 19 to 55 who need family planning services (birth control), the disabled and aged (65 or older), and Medicare recipients who need help paying Medicare premiums. Depending on your financial situation, you will either get health care for free or you may have to pay a small co-payment. If you need health care in Tuscaloosa, Alabama but cannot afford to pay for it, apply to the state's Medicaid program through your local district office in Tuscaloosa.

      Complete the Medicaid application and sign it with dark ink. Make sure all the questions are answered truthfully and correctly.

      Gather supporting documents to submit with your Medicaid application. For pregnant women, families, children under 19 and women age 19 to 55 who require family planning services, the following information and documents must be submitted: Social Security number; proof of identity such as a driver's license or state-issued I.D. card; proof of citizenship such as a U.S. birth certificate; and proof of income such as pay stubs, child support payments, unemployment benefits or the previous year's tax return. If applicable, also provide proof of health insurance and proof of pregnancy.

      Medicare recipients and elderly and disabled applicants seeking Medicaid must submit photocopies of their Medicare card and Social Security card as well as proof of monthly income before taxes. They must also include proof of resources such as bank accounts, stocks, bonds and property; see page 8 under the reference "Medicaid for the Elderly and Disabled" for a complete list of required supporting documents.

      Mail or bring your completed Medicaid application and supporting documents to the Tuscaloosa district office for processing. The office is located at:
