How to File for Medicaid in Florida
If you live in Florida and need help paying for doctor visits, prescription drugs and other health care services, you may be eligible for Medicaid or medical assistance. You must file an application with the Department of Children and Families (DCF) in order to be considered for Medicaid. Your Medicaid eligibility will depend on whether or not you satisfy specific rules set by the state of Florida; your age, income and assets are a few things the DCF looks at when you apply for Medicaid.Things You'll Need

Complete a Florida Medicaid application. Even though Medicaid is funded by both the federal and state government, each state administers its own Medicaid program and sets its own guidelines; therefore, the application process in Florida is different from that of another state's. Florida offers three filing methods for the state's Medicaid program:
Download and complete a paper application by hand; you will need Adobe Acrobat Reader to use this method (see Resources for a free download). Click on the first link under Resources and press "open" when the dialog box pops up---the application will show up on your screen after you press "open." Print out and complete the application.
Or, submit an application online. Navigate to the second link under Resources, click on "Apply for Benefits" and follow the prompts. Press "submit" when you are done with the application.
Or, obtain and complete an application in person at a Customer Service Center or a Community Partner location; to find the nearest office to you, see Resources.

Locate a DCF office near you in Florida. Mail or bring the completed paper application to your desired location for processing.

Gather documents that serve as evidence for the answers you provide on your application. DCF may ask for you to provide them with proof of your income, identity and U.S. citizenship. After DCF receives your application, a case worker will contact you if any documents or proof is required.

Wait for a response from DCF. You will be notified of your Medicaid eligibility within 45 days from the date DCF receives your application.
