How to Apply for Medicaid in Urbandale, Iowa

      Submit your application online or return it in person or by mail to the appropriate Polk County Department of Human Services office.

      Provide documentation to verify your income and resources, such as paycheck stubs, disability award letters, court orders regarding child support and bank statements. See the list of required documents at the end of the application. Your assigned DHS worker will let you know if you need any other documentation. Don't mail original documents to the Department of Human Services. Send copies. The DHS will make copies of your documentation if you deliver the application in person.

      Participate in an interview with a DHS worker as scheduled. The interview may be over the phone or at a Polk County Department of Human Services office. During the interview, a DHS worker will review your application and documentation. Be prepared to wait a month or longer for the state to approve your application. If approved, your benefits are retroactive and will cover bills for up to three months before the date you applied, if you were eligible at that time.
