In Indiana, there are several Medicaid programs available to eligible residents depending on various factors such as the applicant's age and income. For example, Hoosier Healthwise is a Medicaid health plan designed specifically for children age 19 and under, pregnant women and families with limited incomes. Those who qualify for Hoosier Healthwise receive coverage for medical care and services at a low cost or no cost at all. You can get an application by downloading it from the website, by calling your local Family Resources office and requesting that an application be sent to you or by picking up an application in person at an enrollment center nearby (see Resources for application form, office directory and enrollment center locator).

      Read through the instructions on how to fill out the Hoosier Healthwise application form listed under Resources. Complete and sign the application.

      Gather proof of your income (i.e. pay stub) and proof of your child's citizenship or immigration status (i.e. U.S. passport or immigration card or number). Where applicable, you may have to provide documents showing other health insurance your child has and verification from a health care provider that the child you are applying for is pregnant.

      Speak to a Benefit Advocate in person or by phone once a case worker at the Family Resources office or enrollment center has reviewed your application. The Benefit Advocate will help you choose a doctor for your child; selecting a doctor before your application is approved or denied is highly recommended as it will help streamline the application process.
