When you apply for Medicaid in Colorado, you will fill out a general application and submit it to the Colorado Department of Health Care Policy and Financing. This application can be used to apply for both Medicaid and CHP+, the state's health insurance program for children. Colorado Springs is located in El Paso County and, according to Peak Vista Community Health Center's website, there is an abundance of people in the county who are eligible for Medicaid, but who are not covered by Medicaid.       Download and complete the Medicaid application form available at Peak Vista's Medicaid application website. You can either print the application and fill it out by hand, or you can fill in the forms by typing into the form's fields and print it out. You can pick up a Medicaid application at Peak Vista's Registration site at 2840 International Circle in Colorado Springs, or from the Department of Human Services. The Department of Human Services has several locations, and you can find them at the department's website at http://dhs.elpasoco.com/.

      Gather identification and proof of residence, as well as financial records of everyone for whom you are applying for Medicaid. If you or those you are applying for have a Social Security number, you will need to submit proof of it/them as well. Turn in copies of your household's income such as pay stubs and child support, as well as bank account information.
