Find out if you meet Ohio Medicaid's basic eligibility rules. You must be a U.S. citizen or otherwise live in the United States legally, be a resident of Ohio, possess a social security number and have limited income and resources.

Select the program that you wish to apply to. Ohio Medicaid offers several programs for different groups of people.These groups include pregnant women, children and their families; aged, blind and disabled people; and Medicare recipients who need financial assistance with their Medicare expenses. If you don't know which program you should apply to, call the Medicaid hotline at 800-324-8680 or the Franklin County office which serves Columbus residents at 614-462-4000. An agent will help you choose the most appropriate program.

Gather the following information and evidence documents where applicable: proof of income and resources; evidence of U.S. citizenship or other legal status; proof of your pregnancy; other forms of health insurance; and evidence of your age and/or disability. Verify the documents you'll need by looking at your application packet.
